Hello, everyone, hope you all are with good health,

I am writing this as there was a lot of discussion on what should be the solution to nurses’ problems, I know many of you are aware and currently facing or impacted in one way or the other by the ongoing crisis towards the nursing profession.

Optimistically, the best possible solution for the nurse’s problems are always discussed at various forums. Hence, commenting on intellectuals already working towards is not represented here in this article. There are many solutions for the problems faced by the nurses, but most important of it is the attitude of nurses towards there own problems.

1.

We do not react, and hold lamp of patience instead, that is what make us unique in one way

2.

We compare ourselves, the moment we compare ourselves with so called “Doctors” or any health profession will it be, we lost the battle then and there. Many people are doing research regarding this, do not inter-dune with opinions as it suppresses many things.

3.

It is very essential to recognize your strengths and work on them, focus on them, for example many nurses might be exceptionally good in math in their +2, chemistry, physics, computers and so on… Do not stop, please apply that knowledge in nursing find ways to apply. I am glad many do, but most do not so please recognize your strengths.

4.

Now, coming to the most important point “asking right questions”. We have not asked right questions to our seniors, neither to our managers, nor to the government. Decide what you really want in your profession. Is it professional equality or quality? Is it money or conveniences? We want all, but practically its difficult to get there quick, as we know we hold lamps of patience here too. We have to and create an environment where stage by stage we achieve all that we need.

Staying focused on this I have designed a table for all of you. This table is adapted from management of business studies but modified according to our profession. If someone wants to do research on this it is open, where each box is a place of research and exploration.

I request you all to take a look at this table and please fill it according to your institutions, private or government where ever you are, what ever you do. Its very self-explanatory in nature and government organisations can ask their HR’s or finance officers to help you fill it.

What will we achieve by this?

a.

You will understand yourself what question you want and in which region you want to ask.

b.

If you see that no boxes are satisfactorily being achieved, then approach organisational heads at your regional basis and ask for help.

c.

If those in government areas accept this type of payments, then there can be establishment of nurse reserve banks, pocket funds, block funds, and health reserves.

d.

Globally we can setup a unified and dignified labor laws.

As I told earlier each block is a concept and exploration can be done indefinitely and argued extensively. I leave it to the experts in this group and many others to look at the prospects of using this,

ROW – 1

ROW-2

ROW-3

ROW-3.1

Sl. No

Payment method

Fixed payment also termed as one Type payment

Two Type payment but relevant to basic package

“One payment-+” package

1

Payment per visit / procedure

Fee per office visit

Fee per procedure done

2

Payment per patient admission

Payment to a hospital per diagnosis related work/performance

3

Payment per patient period

Payment as per specialist’s health care providers

payment by hospital per diagnosis and treatment combination

Maintenance of Medical standards for early exit.

4

Payment per insured person per period

Caption payment

Budget centered payment

Health maintenance organisational payment. Including Global budget directly into health workers credit

Operationally defining each box to be done according to your concerned institution.

Basic understanding

a.

ROW – 1 – freshers, to _______ definition institutional basis

b.

ROW- 2 – specialist nurses like Nurses in OT, ICU etc_____ definition institutional basis

c.

ROW- 3 and 3.1 – nursing experts, administrators, government organisational representatives ______ definition institutional basis

You can decide who you want to put where and why

Task for common nurses

Try to ask your institution weather what you are paid according to each box mentioned above are you paid or not, if paid how much. Ask these basic questions, in the part of insurance are you getting your chunk of insurance money, because major part is taken by institutions without share.

For those in administration and government agencies:

:

Check weather you have asked this kind of detailed division of payment towards nurses, if 25 thousand is minimum payment you are fixing, then divide that amount in these boxes and see where you failed to fill. Decide the qualification needed for each row, decide how a global budget can reach to a common nurse, find ways to set up a common health insurance for nurses, nursing councils’ improvements and impact factor generation etc. are some questions that can be focused on.
